BOARDING REGISTRATIONS

Thank you for choosing A Country Cat House!

If this is the first time you are boarding with us or if you haven't used our service in over three years, to get started you will need to download the pre-registration form in the format of your preference, fill it out in entirety, save document and send it via e-mail as an attachment.

You can also open, print, fill in, scan and then e-mail the scan to us.

We also prefer vaccination papers to be e-mailed to us, but if that is not possible then fax them one sheet per cat.

We prefer you to send the forms by 3-mail because it is more convenient for our clients and also we can help the earth by saving trees.
